Step One : Choose which membership you would like to join.

Step Two : Create a patient profile.

Medical Clinic : choose the “Gem MD Medical Clinic” when asked for which branch if you are enrolling in the Medical Clinic for primary care.
Weight Management Program : select which membership tier you would like to enroll under. If you are confused or unsure which tier works best for you, please refer to the Weight Management Program page for details on each membership tier. ( click here )
If you are signing up for both the Medical Clinic and Weight Management Program, select “Medical and Weight Management” with the desired tier when asked for branch location.
